Due to the continued pandemic restrictions, we are holding our Annual General Meeting online via Zoom and invite all Members of Corporation to join us.
How to Participate
Participation is voluntary and optional. If you do wish to participate, and you are a Member of Corporation, you can register to attend the virtual AGM or you can appoint a proxy (such as the Chair of the Fields Institute Board of Directors, or someone else of your choosing) to attend and vote on your behalf.
OPTION 1: I will attend
Join our virtual Annual General Meeting on June 24, 2021, at 11 am (EDT). This year’s brief agenda will include standard AGM business items.
OPTION 2: I cannot attend, but I will appoint a proxy
If you can't attend the AGM but still wish to participate, you can appoint a proxy (either the Chair of the Fields Institute Board of Directors, or someone else of your choosing) who will attend and vote on business items at the AGM on your behalf. The online proxy form takes less than five minutes to complete. Completed proxies must be received by June 23, 2021, at 5 pm (EDT). Access online proxy form here.

If you do not wish to use this online form, you may email us a completed PDF version of the proxy form instead. If you prefer, download a PDF version. Please fill it out and send it by June 23, 2021, at 5 pm (EDT). Note that the PDF must be signed, either by e-signature (using software like Adobe Reader) or by scanning a printed, signed PDF.
